What to Bring to
Your Medical Marijuana
California State Law requires that all medical marijuana patients prove valid residency, prior to being awarded a letter of recommendation.
Patients, 18+ years old, on the date of the evaluation appointment; should bring in the following information to the first medical marijuana physician’s evaluation appointment.
1. A VALID PHOTO ID IS REQUIRED – you can present a valid California State driver’s license or California State ID card that includes a photo
New to the state and don’t have a California Drivers License (DL) or State ID (ID)?
You can provide any picture ID with your current DMV receipt stating that you have purchased your California DL or ID.
2. We do not accept credit cards, debit cards or checks.
3. Medical Records & Rx – Bring any medical records or information about current prescription drugs you may take; regularly or from time to time.
You can bring your prescription medicine bottles or a copy of your physician’s prescription.
Your Evaluation Physician will discuss this with you and may request further details.
* Patients that are UNDER THE AGE OF 18 years old must bring in Medical Records, Birth Certificate, and accompanied by their parent(s) or guardian(s), so that either parent(s) or guardian(s) may sign the patient in and approve any further recommendations or treatment, or further care.
